How It Works
Regular visits (monthly to quarterly depending on stability), structured check-ins between visits, lab monitoring, medication adjustments, and clear self-management goals. Your care team includes your physician, our nurse care coordinator, and pharmacy.
What to Expect
Initial visits run 60 minutes to build a complete picture and plan. Follow-ups focus on what's working, what's not, and what to adjust. We track your numbers in plain language and celebrate the wins with you.
Frequently Asked Questions
We're open Monday through Friday from 9:00 AM to 6:00 PM, and Saturdays from 9:00 AM to 2:00 PM. We're closed on Sundays. Emergency appointments may be available outside regular hours.
